This is a sample of the two-page form. Check with your Dean or Supervisor for an updated copy.
Directions:
Course observations will be completed by the Associate Dean, or designee. Designee may be a program manager, adjunct coordinator, or a continuing contract faculty member. Observers will circle yes, no, or to some degree. Comments must be made when no or to some degree is circled.
I. Demonstrates knowledge of subject matter
